LCBA requires that all members upload a picture or scanned copy of their LDWF Charter License.
License images will remain private and will not be shared with the public.
After you have filled out this form and clicked "Create Account," an LCBA staff member will verify your license.
After this verification, you will be granted LCBA membership and full access to the Captains' Dashboard.
